I'm recreating an application form - scan and OCR - because it was sent in hard copy and my handwriting is terrible - so that I can type the information and print it all.

What's the etiquette for their spelling mistakes?

If it's clear from the print-out that you scanned the original, leave as-is. If it looks like you re-made it, fix them. Otherwise they'll think you were the one to make them in the first place.
